1.) Before you start make Wudu and then do some acts of ibadat (pray 2 rakat , say Dhikr ,and etc)
2.) Face the Qiblah. (The Prophet [sallallaahu 'alayhi wa sallam] use to do so when making Dua)
3.) Raise your hands in the air...
Abu Musa Al Ashari (Radhiallahu anhu) said The Prophet (Sallallaahu 'alayhi wa sallam) asked for some water and performed the ablution, and then raised his hands (towards the sky) and said, "O Allah! Forgive 'Ubaid Abi 'Amir." I saw the whiteness of his ARMPITS (while he was raising his hands) and he added, "O Allah! Upgrade him over many of Your human creatures on the Day of Resurrection " (Saheeh Al Bukhari)
4.) And then start with thanking Allah and praising Him. Also acknowledge the benefits and blessings you have been given and thank Allah for them.
Allah says in the Quran:
"Remember Me, and I will remember you, give thanks to Me, and reject Me not" Quran 2:152
"If you would count the blessings of Allah you would not be able to reckon them" Quran 14:34
When was the last time you called out to Allah just to thank Him for everything He blessed you with?
Brothers and sisters you should make dua in times of crises as well as times that are good. Don't just say dua in times of need only.

5.) Know your sins (faults) and seek forgiveness for them.
Even the Prophets use to ask forgiveness from Allah ...
Dua of Ibrahim (Alayhi Salam)
"Our Lord! FORGIVE ME and my parents, and (all) the believers on the Day when the reckoning will be established."
Quran 14:41
Dua of Yunus (Alayhi Salam)
“La ilaha illa Anta [none has the right to be worshipped but You (O Allah)], Glorified (and Exalted) are You [above all that (evil) they associate with You]. Truly, I have been of the WRONG-DOERS."
Quran, 21.87
And look at this Dua of Abu Bakr As-Siddiq (May Allah be pleased with him)
"Oh Allah! Indeed I have WRONGED MYSELF greatly and no one forgives sins except you, so forgive me from your forgiveness and have mercy on me, indeed You And only You are AlGhafoor (the One who covers up sins in vast quantities), AlRaheem (the One who shows mercy in vast quantities)"
And there are so many more examples.
6.) After all that than ask your Lord for what ever you want.
Allah says: “And Your Lord said: Call on Me, I will answer you” Quran 40:60
Allah is Generous and when His servant raises his hands to Him in Dua, He is, shy from returning them empty.
7.) Then send peace and blessings upon the Prophet Muhammad (Sallallaahu 'alayhi wa sallam) . In the beginning of the Dua after praising Allah, in the middle, and that end of your Dua. (and where ever else you get a chance)
Allah Almighty says: "Indeed, Allah confers blessing upon the Prophet, and His angels [ask Him to do so]. O you who have believed, ask [ Allah to confer] blessing upon him and ask [ Allah to grant him] peace." Quran 33:56
8.) After you make you Dua have complete assurance that your Dua will be answered.(Then follow it up with Dhikr)
I would like to also mention that there is things that will cause your Dua to be unanswered. (Haraam income, and etc)
Insh'Allah we will mention them in details some other time.
